Who qualifies for Structured Family Caregiving?
Structured Family Caregiving is for Medicaid-eligible individuals who meet the state’s clinical and functional criteria for long-term care services. Participants must be assessed and approved before services begin.
Does Givers decide who qualifies for the program?
No. Participant eligibility is determined through the state’s Medicaid process. A third party certifies that the participant meets nursing home level of care, and Medicaid makes the final approval decision before services can be provided.
Is this just paying family members for care they were already providing?
No. Structured Family Caregiving formalizes a care relationship that already exists but adds requirements, oversight, documentation, training, and accountability. The caregiver is not simply paid because they are a family member; they are paid to provide authorized support under a Medicaid-approved care plan.
How does Givers make sure care is actually being provided?
Caregivers submit daily care notes documenting the support they provided, including ADLs, IADLs, observations, and changes in condition. Givers leverages proprietary systems and technology to verfiy work and links care records to billing records, and care records must be present for a service day to be billed.
Can caregivers bill for days they were not authorized to provide care?
No. Givers’ mobile application enforces authorized service days. Caregivers cannot submit notes on days that are not authorized in the participant’s person-centered service plan.
Are caregivers screened before they start?
Yes. Caregivers complete required background checks before starting services, including OIG-certified background checks, along with required training and certification.
Are caregivers trained?
Yes. Caregivers complete training and certification before starting service. Givers also provides ongoing coaching and support based on the participant’s needs, caregiver challenges, care notes, and changes in condition.
Does paying family caregivers increase fraud risk?

What happens when something looks unusual?
Unusual records, such as out-of-radius note submissions, missing documentation, reported incidents, or changes in condition, are flagged for review. Givers’ care team follows up to understand what happened and determine the appropriate next step.
